Is a doctor’s referral necessary?

In the state of Indiana, a patient is not required to have a doctor referral for physical therapy. Insurance companies and Medicare may require a referral. If you are seeking insurance coverage please contact us. We will verify your insurance carrier's requirements.

What is Co-Pay?

Co-Pay is the dollar amount assigned by the insurance carrier that must be paid by the patient each visit. Some insurance carriers consider physical therapy under specialist so it may be different than your normal office visit. Please feel free to call us and provide your insurance information. We will verify what your copay is prior to your initial evaluation.

What is Co-Insurance?

Co-Insurance means the patient is responsible for a certain percentage of the amount due for each visit. We will check with your insurance company to find out what the patient responsibility is under your policy. Since the amount due will change based on what is done each visit we will apply your percentage to our lower self pay cost each visit to avoid a large outstanding at the end. Once the explanation of benefits are received from the insurance company the amount will be calculated which may result in additional payments or a credit to the patient.

What is a Deductible?

Deductible is the amount the patient is responsible to pay before the insurance company will start to pay, anywhere from $200 and up. If your deductible has not been met you will be responsible to pay each visit until it has been met. We will check with your insurance company and let you know if you have a deductible, the amount and if any of it has been met.

When are the Co-Pay, Co-Insurance, and Deductible monies collected?

All co-Pays, co-Insurance payments, and deductible payments are collected each visit. If you have a deductible that has not been met, you first pay the deductible amount. Once the deductible is met, you pay the coinsurance amount per your benefit plan.
